uh, switch on SAS, launch the rocket, when on 100 m/s tilt slightly over, let SAS do the rest. Correct only slightly and not too much, don't go over the green prograde marker in the navball

If you add fuel, try adding it to the top stage ;)

Distribute fuel so that stages with higher Isp have equal or larger mass ratios than stages with lower Isp

Usually for a Mun mission I do three stages.

One lift stage, which gets me almost into orbit, one transfer stage to get me into orbit and to Mun and then the landing and return stage.

for Mun return, if done correctly, you don't even need a heat shield and if you bring one, you can put the slider waaaayyyyy down (and save weight)

i returned from mun orbit without heatshield already. then i tried one but the slider makes little difference. so i left it out again
