this is a really beautiful nerve diagram
<Djohaal>
protip cardiac bundles are not nerves
<SilverFox>
oh? is it cardiac muscle bundle?
<Djohaal>
conductive fibers for the lack of a better term
<SilverFox>
i mean, it looks like nerves, and I haven't been educated on any other bundle type
<Djohaal>
they are special fibres which have direct cell-to-cell conduction
<Djohaal>
no synapses inbetween
<SilverFox>
do these conductive fibres operate any differently than nerves?
<SilverFox>
ahh
<SilverFox>
how beautiful
<SilverFox>
why cant we have instant comms like this for the brain?
<Djohaal>
synapses aren't just connections. They do a lot of processing if you consider the brain a computer
<Djohaal>
if you consider them as diodes, that's a start (synpatic conduction is one-way)
<Djohaal>
but then you have a host of neurotransmitters and proteins that influence how easily or not a synapse will trigger
<SilverFox>
right, isn't there also an additive effect with multi-dendratic ones?
<Djohaal>
so each synapse is a very complex processing unit. And the brain has hundreds of trillions of synapses
<SilverFox>
right, I see now, I forgot about all that for a bit
<SilverFox>
"This treatment is killing her faster than the cancer is" that gave me the chills

<SilverFox>
oh Djohaal, chilling question, how young was the youngest patient you lost?
<Djohaal>
35 I think
<SilverFox>
wow, that's rather not-young
<Djohaal>
I never worked on trauma ward or pediatrics
<Djohaal>
just very short internships, not enough to lose a patient
<Djohaal>
that I remember at least
<SilverFox>
you did a peds rotation though right?
<Djohaal>
yup
<SilverFox>
and not one child died during that?
<Djohaal>
I did tend to a little girl with 6 years that died a few months after
<SilverFox>
how long are rotations?
<Djohaal>
yes, a lot of patients in my indirect care during medschool died, I mean as "my" patient after I graduated and was alone
<SilverFox>
I meant moreso your care, like you as a doctor were assigned to the case or picked up the case and then they met the reaper
<Djohaal>
first patient I lost after graduating was a 35-ish old man. He had a random ecography that suggested something fucky on his liver. I had a strong suspection of liver cancer (hepatitis C, which he also discovered by then) he went to get a liver transplant, but died on post op.
<Djohaal>
I was the GP in the story though, he died in the hospital
<Djohaal>
I was fortunate enough to not have any patient die in front of me
<SilverFox>
oh, are those questions on hypothetical shit they do on med shows a real thing? where all the doctors/med students go around like penguins following the attending and answering questions
<Djohaal>
my year as family doctor, dozens of old people. The reaper was close by me that year, sadly
<SilverFox>
how sad
<Djohaal>
depends on your viewpoint. Most were terminal patients in a lot of suffering
<SilverFox>
ahh
<SilverFox>
I dont want to say "good", but good
<Djohaal>
but yeah, I ended my year there with way less terminal old people than I started. Learned a bit about palliative care too
<SilverFox>
at least they dont suffer anymore
<Djohaal>
and yes the penguin part is miserably true. I hate it
<SilverFox>
sounds fun for the first day or two
<SilverFox>
I get competitive when im not being lazy
<Djohaal>
it becomes a miserable human circus
<SilverFox>
oh?
<SilverFox>
give the deets
<Djohaal>
you have: A: the overacheiving girl who wants to be right 99% of the time B: the tech savy kid with pubmed on his fingers questioning everything C: The asshole professor that terrorizes someone whenever they do a slight slip D: the narcisitic adjunct doctor who wants to show off and shag the intern girls
<Djohaal>
so on
<SilverFox>
these stereotypes are pretty great
<SilverFox>
what do you mean "so on" gib moar. these are great
<SilverFox>
which stereotype did you fall under?
<Djohaal>
the kid with orthostatic hypotension trying to not faint, legs burning with pain and wondernig why the fuck those rounds have to be done standing up in the corridor
<Djohaal>
also annoyed at the fact the professors discuss about terminal diagnosis in front of the patients. I mean we get a lot of simple and uneducated folk, but they paid attention and knew it was no good
<SilverFox>
nice

so during death, muscles are still tense because they have to be told to relax. So how do they relax some time after death? is it just the lysing of the muscle?
<Djohaal>
yup
<SilverFox>
figured as much, anymore to it?
<Djohaal>
the active portion of muscle contraction is muscle fibres detaching to move a step
<Djohaal>
so you get locked when there's no energy to do it
<Djohaal>
IIRC the release is when lysis starts to take place, the proteins don't stay boudn forever
<SilverFox>
and lysis happens after cell stops getting sufficient nutrients and the lysosomes burst?
<Djohaal>
general cell death, pretty much
<SilverFox>
we werent educated on "general cell death"
